Immediately: Billing and care coordination staff must identify all Health Net Wellcare Dual Liberty Amber D-SNP patients and verify their Medi-Cal managed care plan. Staff must coordinate all benefits, discharge planning, disease management, and care management across both Medicare and Medi-Cal. Update patient intake procedures to capture Medi-Cal plan information and establish workflows to assist patients with scheduling appointments at their Medi-Cal plans, arranging transportation, and documenting follow-up care.
