Summary
Health Net Medi-Cal has renamed Initial Health Assessments to Initial Health Appointments (IHA) and eliminated IHEBA and SHA form requirements. Providers can now earn incentives for completing IHAs within 120 days for new Medi-Cal members, and must help patients complete Health Information Forms (HIF) during visits.

Immediately: Update documentation workflows to reflect that Initial Health Assessments are now called Initial Health Appointments (IHA). Stop requiring IHEBA and SHA forms for new Medi-Cal members. During IHA visits, help patients complete Health Information Forms (HIF) - keep copy in member chart and send original to Medi-Cal Care Management at 4191 E. Commerce Way, Sacramento, CA 95834. Track IHA completion within 120 days of member enrollment for potential incentive payments.
