Summary
CalViva Health has updated Medi-Cal Initial Health Assessment requirements, renaming it to Initial Health Appointment (IHA) and adding financial incentives for completing IHAs within 120 days for new members. The IHEBA and SHA forms are no longer required, but providers must help patients complete Health Information Form (HIF) during IHA visits.
Action Required
Immediately: Update documentation workflows to reflect IHA name change from Initial Health Assessment to Initial Health Appointment. Train front desk and clinical staff to help new Medi-Cal members complete HIF forms during IHA visits. Keep copies of completed HIFs in member charts and mail originals to Medi-Cal Care Management at 4191 E. Commerce Way, Sacramento, CA 95834. Stop using IHEBA and SHA forms for new Medi-Cal members.
