Wait for rate information expected in early 2023. Participating physician groups should note they are NOT eligible for this incentive. Help patients complete Health Information Forms (HIF) during IHA visits and send completed forms to Medi-Cal Care Management at 4191 E. Commerce Way, Sacramento, CA 95834.
