Summary
CalViva Health has updated the mailing address for Health Information Forms (HIF) submissions. Primary care providers must now send completed HIF forms to a new P.O. Box address in Farmington, MO instead of the previous address.
Action Required
Immediately: Update all HIF submission processes to send forms to the new address: Medical Management Notifications, P.O. BOX 2010, Farmington, MO 63640-9706. Front desk and billing staff must update any stored addresses, mailing labels, and submission workflows. Continue to keep copies in member charts and ensure forms are submitted within 90 days of member assignment to PCP.
