Health Net will be randomly requesting medical records from physicians and PPGs from January 1-May 3, 2024 for HEDIS quality measures using their contracted vendor Datafied. Providers must submit requested records within 7 days or complete a Certificate of No Record form.

January 1-May 3, 2024: All staff must respond to Datafied requests within 7 days by either submitting requested medical records or completing the Certificate of No Record (CNR) form. Front desk staff should confirm contact information when Datafied calls. If unable to meet 7-day deadline, contact Datafied at 714-666-0951 immediately. Failure to respond will result in follow-up requests from Datafied.
