Summary
Health Net has established a process for Los Angeles County patients who were transitioned to Molina Medi-Cal effective January 1, 2024, but want to return to Health Net. Providers must complete a specific PCP/PPG Change Form while the patient is in the office and submit it with a 15 business-day processing time.

Immediately: Front desk staff must identify Los Angeles County patients transitioned from Health Net to Molina who wish to return. Complete the Request for PCP/PPG Change Form (available in English and Spanish) while patient is in office, checking the Health Net box. Patient must acknowledge and sign form with future effective date. Submit via fax or email to Health Net Medi-Cal Member Services. Allow 15 business days for processing.
