Box 14621, Lexington, KY 40512-4621) and Payer ID 22771 for services on/after Jan 1, 2024. Use MHN address for services prior to Jan 1, 2024. Split claims with overlapping dates into separate submissions. Register for Health Net provider portal account for eligibility checks and claims status.
