Summary
Starting March 1, 2025, Health Net Medi-Cal members requesting non-emergency medical transportation (NEMT) must contact their provider directly instead of Modivcare. Providers must complete and fax PCS forms directly to Health Net's Care Ride Unit for prior authorization at 833-701-0051, not to Modivcare.
Action Required
Before March 1, 2025: All providers must update patient instructions to inform Health Net Medi-Cal members they must contact the provider (not Modivcare) for transportation requests. Download updated PCS forms from Health Net's Provider Library. When members request NEMT, complete and sign the PCS form and fax directly to Health Net at 833-701-0051 for prior authorization. Do not send to Modivcare. Update front desk staff training on new NEMT request workflow.
