Summary
Starting March 1, 2025, Health Net Medi-Cal members must request non-emergency medical transportation (NEMT) through their physician instead of contacting Modivcare directly. Providers must complete and fax PCS forms directly to Health Net at 833-701-0051 for prior authorization, not to Modivcare.
Action Required
By March 1, 2025: Front desk staff must inform Medi-Cal members they need to contact their physician for medical transportation requests. Providers must complete and sign PCS forms when members request transportation. Billing team must ensure all PCS forms are faxed to Health Net at 833-701-0051 for prior authorization, NOT to Modivcare.
