Summary
Health Net is changing ADT (admit, discharge, transfer) reports from Excel to CSV format effective June 6, 2025. The new CSV format will include additional data fields and improve reliability for tracking patient status changes and care coordination.
Action Required
Before June 6, 2025: IT/Operations team should verify ability to process CSV files instead of Excel files if your practice currently receives Health Net ADT reports through Cozeva or SFTP. Contact your Health Net provider engagement account executive if you want to start receiving these reports for care coordination and quality measure tracking.
