Health Net requires providers to complete a survey by June 13, 2025 to indicate whether they offer gender-affirming services and want this information displayed in provider directories. This is a new requirement from the Department of Health Care Services effective February 14, 2025.
Action Required
By June 13, 2025: Providers must complete Health Net's gender-affirming services survey at Health Net Gender-Affirming Services website. Have national provider identifier number, taxpayer identification number and Health Net assigned ID available. Survey takes less than 10 minutes and indicates whether your practice offers gender-affirming services and if you want this displayed in directories. Failure to respond may result in incomplete provider directory listings.
