By December 31, 2025: All physicians and providers treating Dual Eligible Special Needs Plan (D-SNP) and Chronic Condition Special Needs Plan (C-SNP) members must complete the online Special Needs Plan Model of Care Annual Training. Access training at Provider Training Webinars and Other Provider-Related Resource under Required Trainings. Submit voluntary attestation after completion. Note: Training completion is auditable by CMS.
