Summary
Ohio Medicaid has released a new Case Management Referral Form for use by care providers referring members to Case Management or Continuity of Care services through Anthem Blue Cross and Blue Shield. Providers must complete and submit the form via email to OHPHCaseManagement@anthem.com to ensure timely member outreach and appropriate coordination of services.
Action Required
By June 1, 2026: Providers and clinical staff must obtain and familiarize themselves with the new Ohio Medicaid Care Management Referral Form (available as downloadable PDF). When referring Ohio Medicaid members for Case Management or Continuity of Care services, complete the form with accurate and complete information, then email to OHPHCaseManagement@anthem.com. Update internal referral processes and staff training to reflect the new form requirement. Contact Provider Services at 844-912-1226 with any questions. Failure to use the new form may delay member outreach and care coordination.
