Participating Physician Groups (PPGs) must now notify CalViva Health when any contracted hospital terminates from their network, effective January 1, 2026. This includes providing detailed termination information at least 180 days in advance and completing standardized forms 120 business days before termination to allow for required regulatory filings.
Action Required
By January 1, 2026: PPGs must establish process to notify assigned Provider Engagement network specialist immediately when hospital terminations are known or anticipated, at least 180 days in advance. Complete standardized termination forms provided by network specialist at least 120 business days before termination, including hospital termination notice, effective dates, enrollee notification materials, and COC approach. Failure to provide timely notification may result in corrective action affecting delegation status.
