By January 1, 2026: Provider network administrators must establish process to notify assigned Health Net Provider Engagement network specialist immediately when hospital terminations are known or anticipated, at least 180 days in advance. Prepare to submit standardized termination form at least 120 business days before termination including hospital termination notice, effective dates, enrollee notification materials, COC approach, and scope of impact. Failure to provide timely notification may result in corrective action and impact delegation status.
