Summary
HealthNet California has introduced a new Housing Support Plan (HSP) Template that must be used when submitting prior authorization requests for housing deposits, transitional rent, and housing navigation services. Requests submitted without the completed HSP template when required will be automatically denied.
Action Required
Immediately: Billing team must download the Housing Support Plan Template from CalAIM Resources for Providers page under Forms & Tools > Community Supports. Update prior authorization workflow to include completed HSP template for all housing support requests (deposits, transitional rent, housing navigation services). Keep copies in member files for audit purposes. Failure to include HSP will result in automatic denial.
