Summary
Connecticut Medicaid has transitioned School Based Child Health (SBCH) providers from paper-based enrollment to mandatory online enrollment/re-enrollment through their web portal. Providers must now use the Hewlett Packard Enterprise Provider Enrollment Wizard and can no longer submit paper applications.
Action Required
SBCH providers in Connecticut must: 1) Use the online Provider Enrollment Wizard at www.ctdssmap.com for all new enrollments and re-enrollments (no more paper applications), 2) Complete applications in their entirety as they cannot be saved for later completion, 3) Submit re-enrollment applications immediately upon receiving notification (6 months before due date) to allow processing time, 4) Subscribe to email notifications for training workshops. Failure to re-enroll before the due date will result in provider agreement termination.
