Summary
Connecticut Medicaid is changing suspended claims with EOB codes 3015 (Care Plan Required) and 3016 (Service Not Covered Under Care Plan) to denied status, effective April 26, 2016. Home care, personal care assistance, and home health providers must contact designated Access Agencies to resolve care plan issues for these denied claims.
Action Required
Immediately: Billing team must identify all claims with EOB codes 3015 and 3016 that will appear as denials on April 26, 2016 Remittance Advice. Contact appropriate Access Agency (CCCI, SWCAA, AASCC, or WCAA) with required client information including provider name, client EMS number, service dates, service type, and frequency to resolve care plan issues and resubmit claims.
