Summary
Effective May 11, 2016, Claim Adjustment Reason Codes (CARC) B5 will be eliminated and replaced with new codes in Electronic Remittance Advice (ERA) processing. Two specific CARC/RARC combinations are changing: B5/N584 becomes 272/N584, and B5/N640 becomes 273/N362.
Action Required
By May 11, 2016: Billing team must update ERA processing systems and denial management workflows to recognize new CARC codes 272 and 273 instead of eliminated CARC B5. Review the CAQH website for complete code combination changes. Update denial tracking and appeals processes to handle the new code combinations.
