Summary
Connecticut DSS has established procedures for removing Hospital Lock-In status and using Medicaid prescription vouchers (Form W-1069) for individuals released from correctional institutions. Pharmacies must contact the DSS Pre-Release Unit to process prescriptions for these patients and can accept vouchers that guarantee Medicaid reimbursement for up to 30 days of medications.

Train staff to accept W-1069 vouchers within 5 days of authorization date and contact CMHC pharmacy at (860) 679-8020 for discharge prescriptions. Update billing procedures to process voucher claims electronically through Hewlett Packard Enterprise.
