Summary
Connecticut Medicaid requires hospitals and FQHCs to sign and return a Provider Enrollment Agreement Addendum to participate in Project Notify, a health IT initiative that automatically sends admission/discharge notifications to primary care providers and specialists when Medicaid patients are admitted to or discharged from hospitals.
Action Required
By December 31, 2016: Hospitals and FQHCs must review, sign, and return the Addendum to Provider Enrollment Agreement for Project Notify and Other Health IT Initiatives to minakshi.tikoo@ct.gov. If unable to meet any requirements, providers must still sign but include a detailed letter explaining which requirements cannot be met and a compliance plan.
