For HUSKY B pregnant members, add secondary diagnosis codes O99.330-O99.333. Family planning clinics must use HCPCS codes S0106, S5001, S4991 with NDC codes and quantity of 1. Outpatient hospitals must use revenue center code 914 with counseling codes. FQHCs must bill counseling codes with T1015 encounter code.
