Action Required
Immediately: Update billing system to use new HCPCS codes Q9985 or Q9986 instead of J1725 for hydroxyprogesterone caproate injections. Use Q9984 for Kyleena instead of C9399. Use G0297 for low dose CT lung cancer screening. Submit prior authorization requests for Eteplirsen (C9484) and Nusinersen (C9489) to Medical Director via fax to (860) 424-4822. Update system to recognize new EOB codes 6000, 9100, 9101, and 9102 starting August 1, 2017. Claims without proper coding will be denied.
