Use G0297 for low dose CT lung cancer screening. Submit prior authorization requests for Eteplirsen (C9484) and Nusinersen (C9489) to Medical Director via fax to (860) 424-4822. Update system to recognize new EOB codes 6000, 9100, 9101, and 9102 starting August 1, 2017. Claims without proper coding will be denied.
