Obtain prior authorization from CHNCT for hearing aids, assistive listening devices, and DME items $25+ by faxing PA form to (203) 265-3994. Bill all items at actual acquisition cost. Verify ordering practitioners are enrolled in CMAP provider network. Claims will be denied if proper billing channels and authorization requirements are not followed.
