Action Required
Effective January 1, 2018: Billing team must obtain prior authorization for Q2040 (Tisagenlecleucel CAR-T therapy) by calling CHNCT at 1-800-440-5071. Replace expired codes C9484 with J1428 for Eteplirsen billing and C9489 with J2326 for Nusinersen billing. Download updated CMAP Addendum B from ctdssmap.com to verify all HCPCS code changes. Claims with old codes will be denied after December 31, 2017.
