Summary

Ongoing requirement: Providers serving Connecticut Autism Waiver clients must submit written progress reports every 6 months and annually to the client's DSS Case Manager. Use the provided Individual Plan Review form to document service hours, outcomes progress, and any significant changes in client condition.
