For Connecticut Medicaid providers expanding practice locations: Use the new online portal at www.ctdssmap.com by selecting Provider > Add Alternate Svc Loc Address instead of calling DXC Technology. Master users must assign the 'Submit Applications' role to clerks who will handle location updates through Clerk Maintenance. Submit required follow-on documents within 60 days to avoid application denial.
