Revised Medicaid (HUSKY) Spend-down Procedures pdf

Immediately: Update spend-down submission procedures for Connecticut HUSKY patients. Mail all spend-down medical expenses to DSS ConneCT Scanning Center, P.O. Box 1320, Manchester, CT 06045-1320 or fax to (860) 643-6999. Download and use the new Spend-down FastLink cover sheet from the DSS website for each submission, completing client's full name, Medicaid ID, and ImpaCT case number. Use separate cover sheets for each client and only use original prints (do not copy as barcode will degrade). Failure to follow new procedures may result in expenses not being properly credited to patient spend-down accounts.
