Effective July 1, 2018, the Connecticut HUSKY Health program (Medicaid) will no longer cover non-surgical permanent birth control devices due to FDA safety restrictions. HCPCS code A4264 will be restricted to exclude the specific implantable contraceptive device, and CPT code 58565 for hysteroscopy sterilization will no longer be payable.

By July 1, 2018: Billing team must stop billing HCPCS code A4264 for permanent implantable contraceptive intratubal occlusion devices and stop billing CPT code 58565 for hysteroscopy sterilization procedures for HUSKY Health patients. Update billing system to deny these codes for Connecticut Medicaid. Continue using A4264 only for other covered birth control devices like Falope rings and filshie clips. Claims using restricted codes will be denied.
