Immediately: Billing team must stop billing HCPCS code A4264 for non-surgical, non-hormonal implanted birth control devices and CPT code 58565 for hysteroscopy sterilization for Connecticut HUSKY Health patients with dates of service July 1, 2018 and forward. Update billing system to reject these codes for HUSKY Health. Continue using A4264 for other covered birth control devices like Falope rings and filshie clips. Claims for discontinued devices will be denied.
