Action Required
Before September 1, 2018: Billing team must stop submitting D1354 claims directly to DXC Technologies. All D1354 services now require Special Pricing Prior Authorization (SPPA) through www.ctdhp.com or hard copy submission to Bene Care Dental Plans. Update billing workflow to obtain SPPA first with documentation of medical necessity, radiographs or intraoral photographs. Direct submissions without prior authorization will be denied.
