Summary
Anthem Blue Cross and Blue Shield is promoting electronic submission of corrected claims for Federal Employee Program (FEP) members through Availity Essentials or EDI. Providers must use specific frequency codes (6, 7, or 8 for professional claims; 0XX6, 0XX7, or 0XX8 for institutional claims) and include the payer control number from the original claim to indicate corrections, replacements, or voids of previously adjudicated claims.
Action Required
By April 30, 2026: Billing team must train all staff on the corrected claims process for FEP members. Update internal procedures to require use of Availity Essentials or EDI when submitting corrected, replacement, or void claims. Ensure staff obtains the payer control number (ICN/DCN) from the 835 ERA or Remittance Inquiry before resubmitting claims. Verify practice management software supports frequency codes 6, 7, and 8 (professional) or 0XX6, 0XX7, and 0XX8 (institutional) for claim corrections. Contact software vendor or clearinghouse if modifications are needed. Stop using paper/mail methods for corrected FEP claims to reduce processing delays. Document the process in billing workflows and encounter templates.
