Two provider types are available: MHW Service providers (no NPI required) and ALSA providers (require NPI with taxonomy 310400000X).

By September 1, 2019: Mental Health Waiver providers in Connecticut must complete online enrollment at www.ctdssmap.com as either MHW Service or ALSA providers. ALSA providers must obtain NPI with taxonomy 310400000X. All providers must obtain credentialing letter from Advanced Behavioral Health and submit with Application Tracking Number to DXC Technology. Set up secure web account after receiving Welcome and PIN letters. Subscribe to email communications for program updates. Failure to enroll will prevent reimbursement for MHW services.
