Action Required

Action needed
Immediately: FQHC billing teams must update billing procedures to include HCPCS code T1015 with all medical service claims along with valid CPT/HCPCS procedure codes. Ensure all claims include performing provider NPI numbers. For nutritional services by registered dieticians or acupuncture services by licensed acupuncturists, list an enrolled FQHC health professional as the performing provider. Claims without proper performing provider information will be denied with EOB codes 0231, 1011, or 1010.
