Summary
Connecticut Medicaid (CMAP) will no longer accept paper Trading Partner Agreement forms effective July 1, 2019. All new trading partner enrollments and updates must be submitted electronically through the www.ctdssmap.com web portal.

For Connecticut providers only: If you need to enroll as a new trading partner or update existing trading partner agreements with Connecticut Medicaid, you must use the online portal at www.ctdssmap.com. Go to Trading Partner > Trading Partner Enrollment/Profile and follow the prompts. Refer to Chapter 10 of the Provider Manual for detailed instructions. Do not submit paper forms as they will be returned or destroyed.
