Connecticut Medicaid updated their Genetic Testing Prior Authorization form effective September 1, 2019. The new form must be used for all genetic testing requests except Whole Exome Sequencing, Whole Genome Sequencing, and Non-invasive Prenatal Testing, which continue using their specific forms.

This policy is from 2019 and should already be implemented. If still submitting genetic testing prior authorizations for Connecticut Medicaid patients, ensure using the current Genetic Testing PA form (not the forms for Whole Exome/Genome Sequencing or Non-invasive Prenatal Testing).
