Connecticut Medicaid (CMAP) is implementing new coverage guidelines for varicose vein treatments of the lower extremity, retiring the previous Stab Phlebectomy and Sclerotherapy policy. All varicose vein treatments now require prior authorization using updated medical necessity criteria.

Action Required

Action needed
Before November 1, 2019: Billing team must ensure all Connecticut Medicaid varicose vein treatments require prior authorization using the Outpatient Prior Authorization Request Form from www.ct.gov/husky. Providers must submit clinical documentation supporting medical necessity with each PA request. Contact CHNCT at 1-800-440-5071 for PA questions. Failure to obtain prior auth or provide adequate documentation within 20 business days will result in claim denials.
