When answering 'no' to any clinical questions on Opioid PA Forms for HUSKY members, scan and email completed forms with letters of medical necessity to Rx.LMN@ct.gov instead of faxing. Download updated Opioid PA Form from www.ctdssmap.com. Continue faxing forms where all clinical questions are answered 'yes'.
