By September 4, 2020: Eligible providers enrolled in Connecticut Medical Assistance Program must log into their CMAP secure provider Web portal accounts, download the Notice of CRF Payment Standard Form, sign electronically, complete the CRF Payment Cost Report Form if applicable, and submit to DSS.
