Summary

This applies to home health, behavioral health, and certain residential treatment providers enrolled in the Connecticut Medical Assistance Program.
Action Required
By September 30, 2020: Eligible Connecticut Medicaid providers (home health, behavioral health clinics, residential treatment facilities, methadone clinics) must log into their CMAP secure Web portal accounts, download and electronically sign the Notice of CRF Payment Standard Form, and if applicable complete the CRF Payment Cost Report Form. Submit all forms to DSS before the extended deadline to receive coronavirus relief funding.
