Action Required

Action needed
By February 1, 2022: HCBS providers must 1) Execute a data sharing agreement with Connecticut's HIE (Connie) by completing the online Interest in Connecting Form and signing the DocuSign agreement, and 2) Enroll supervisors and managers in Trauma Informed Care Racial Equity Lens Initiative training by November 30, 2021 and complete first course by February 1, 2022. Submit training enrollment form to dsslearncentersupport@ct.gov. Failure to complete both requirements will result in loss of 1% supplemental payment eligibility.
