Summary

Starting February 2, 2022, Connecticut Medicaid (HUSKY A/B/C/D) will cover Omnipod Pods and VGO insulin delivery devices through pharmacy point-of-sale with prior authorization, in addition to existing DME coverage. These products will be subject to the Diabetic Preferred Product List when billed as pharmacy claims.

Before February 2, 2022: Billing team must obtain new Prior Authorization forms from www.ctdssmap.com under Pharmacy section for Omnipod and VGO devices when billing as pharmacy claims. Review updated Diabetic Supplies Preferred Product List for preferred insulin delivery products. Do not submit pharmacy claims for Omnipod Personal Diabetes Manager (PDM) as it's provided free by manufacturer.
