Summary
Connecticut Medicaid (CMAP) now requires application fees of $631 for institutional providers enrolling or re-enrolling as of March 2, 2022. Individual physicians and non-physician practitioners are exempt, but fees apply to hospitals, facilities, and other institutional providers unless they're already enrolled in Medicare or paid the fee to CMS.
Action Required
Institutional providers in Connecticut: When submitting new CMAP enrollment or re-enrollment applications, prepare to pay the $631 application fee unless exempt (individual physicians are exempt, or if already enrolled in Medicare/another state Medicaid). Contact Provider Assistance Center at 1-800-842-8440 if fee notification is received. Note that applications cannot be processed until fee is paid, and fees are non-refundable.
