Summary
Connecticut Medicaid (MAP) now covers insulin pumps (Omnipod and V-Go devices) through pharmacy point-of-sale with prior authorization effective February 2, 2022. Additionally, at-home COVID-19 tests are covered through pharmacies with limits of 8 tests per 30-day period without prior authorization.
Action Required
For providers serving Connecticut Medicaid patients: 1) Update billing systems to process insulin pump supplies (Omnipod Pods and V-Go devices) through pharmacy POS with prior authorization requirements. 2) Obtain Insulin Pump PA Form from www.ctdssmap.com under Pharmacy section. 3) Note that Omnipod Personal Diabetes Manager (PDM) is provided free by manufacturer and should not be billed to MAP. 4) For pharmacies: Process at-home COVID-19 tests through POS system, limit 4 test kits (8 tests) per member per 30 days, can use pharmacist NPI as prescriber if no prescription provided.
