By April 5, 2023: Pharmacy staff should note that Connecticut Medicaid patients can now receive up to 28 days supply of Hepatitis C medications per fill (increased from 14 days). Continue using current Hepatitis C Prior Authorization form until April 5, then download updated PA form from www.ctdssmap.com. No changes to clinical criteria or billing processes required.
