Summary
Connecticut Department of Social Services is issuing Value Based Payment for Q2 2023 to Home Health Agencies and Home/Community Based Services providers. Providers must complete and return a form by July 12, 2023 to receive payment, agreeing to future Health Information Exchange requirements when the portal becomes available.
Action Required
By July 12, 2023 at noon: Home Health Agencies and HCBS providers must print, complete, and email the attached form to DSSARPVBP@ct.gov including Provider ID/AVRS ID. Must agree to complete HIE Administrator identification and training when portal becomes available. Failure to complete HIE requirements within 2 months of portal availability may result in payment recoupment.
