Summary
Connecticut DSS requires Home and Community Based Services (HCBS) providers to complete three requirements by October 15, 2023 to receive a 2% rate increase through value-based payments for services rendered July 1 - October 31, 2023. Providers must submit an HIE Administrator form, complete racial health equity training, and attend person-centered care learning collaboratives.
Action Required
By October 15, 2023: HCBS providers must: 1) Complete and submit the 'Agreement to Identify HIE Administrator' form to DSSARPVBP@ct.gov, 2) Register for and attend one 1-hour Racial Health Equity Learning Collaborative session (Sept 26, 28, Oct 3, 5, 6, or 10), 3) Register for and attend one 1-hour VBP #3 Learning Collaborative on Communication and Documentation (Sept 25, 27, 29, Oct 2, 4, or 11). New providers must also complete additional CONNIE HIE agreements and staff trauma-informed care training. Failure to meet benchmarks will result in loss of 2% payment increase.
