By October 15, 2023: Connecticut HHA providers must: 1) Complete and submit the 'Agreement to Identify HIE Administrator' form to DSSARPVBP@ct.gov, 2) Register for and attend a 1-hour Racial Health Equity Learning Collaborative (multiple dates available Sept 26-Oct 10), 3) Register for and attend a 1-hour VBP Communication and Documentation Learning Collaborative (multiple dates available Sept 25-Oct 11). New providers must also complete additional requirements including CONNIE data sharing agreements and staff training modules. Failure to meet requirements will result in ineligibility for 2% payment increase on claims from July 1-October 31, 2023.
