Summary
Connecticut Medicaid is temporarily setting EOB code 3328 'Confirmed Visit Units are Exceeded' to post and pay for waiver home health claims with dates of service 4/1/2024 and forward. All providers should rebill previously denied claims with this EOB code while the system issue is being resolved.

Immediately: Home health billing teams must rebill all previously denied claims that received EOB code 3328 'Confirmed Visit Units are Exceeded' for dates of service 4/1/2024 and forward. Monitor the Connecticut Medical Assistance Program (CMAP) website regularly for updates on when this temporary fix will be reversed and normal edits reactivated.
